Alzheimer's Services of the East Bay (ASEB) - Oakland

Alzheimer's Services of the East Bay (ASEB) programs are designed to help the frail elderly and disabled to continue to live at home, and/or with their loved ones for as long as possible. The programs aim to enhance personal independence; maximize physical capabilities; provide support services and respite for families and caregivers; and enhance the quality of life.

Services available for both programs include nursing (monitoring of health status, assistance with medications, personal care and health education), rehabilitation (physical, occupational and limited speech therapy), social services (information and referral, counseling, advocacy, individual support services to participants and families), nutrition (regular and specific diets; a morning snack, mid-day meal, nutrition counseling and education) and recreation (recreational, creative and social activities for varying cognitive levels).

To be eligible you must be at least 18 years of age, be disabled, have a health problem, and/or need rehabilitation services to ensure your continued independent living. Referrals to the programs can be made by physicians, nurses, social service agencies, hospitals, friends or family members. The programs serve Oakland, Alameda, South Berkeley, Piedmont and Emeryville. Transportation is available. Fees are covered by MediCal or by private payment on a sliding scale.
