Hong Fook Adult Day Health Care - Harrison Street

Photo: A staff member gentle helps a woman negotiate her walker. Hong Fook ADHC - Harrison Street, is the second adult day health care program developed by Family Bridges, Inc. (formerly Oakland Chinese Community Council, Inc.). The center, which opened in May 2000, is a multi-disciplinary program of health and social services serving at-risk frail and disabled Asian adults in Alameda County. Family Bridges has always taken pride in being able to serve the county's monolingual Chinese-speaking community. With this second ADHC, vigorous outreach is now being made to the Vietnamese-speaking community in order to provide the same linguistically and culturally appropriate services to this under-served adult population.

Photo: A woman uses arm and shoulder strengthening equipment against a wall. With the goal of preventing premature institutionalization, the center offers a range of clinical and supportive services. These are provided by a registered nurse, a medical social worker, a registered dietitian, counselors, along with physical, occupational and speech therapists, and include health assessments and regular monitoring, therapies to improve daily overall functioning, recreational and social activities, nutritional counseling and assistance with accessing community resources. Most of the clinical staff and all of the program/health care aides are bi-lingual and bi-cultural in Chinese/English or Vietnamese/English. Translation services are readily available for non-bilingual staff. Meal service is also oriented to Asian tastes and preferences.
