Hong Fook Adult Day Health Care Center
Hong Fook Adult Day Health Care Center's mission is to enable frail seniors and disabled adults to remain in the community with their loved ones. Loc1ated in the heart of Oakland's Chinatown, the center serves a rich cultural mix of Chinese, Korean, Filipino, Laotian, African American and Caucasian participants. Bilingual staff are employed in key positions to ensure that monolingual participants are clearly understood, both linguistically and culturally. The Center is one of seven programs operated by Family Bridges, Inc., which aims to empower the most vulnerable members of the East Bay community -- especially the elderly, newcomers and those with limited English language skills.

At the center, a talented multidisciplinary team -- including a registered nurse, a medical social worker, a registered dietitian, along with physical, occupational and speech therapists -- provides holistic health care. The program promotes independent living for each of its participants and addresses the obstacles often facing those individuals. The clinical and supportive services provided include health assessments and regular monitoring, therapies to improve daily overall functioning, recreational and social activities, nutritional counseling and assistance with accessing community resources.

Hong Fook ADHC Center is located in the renovated Ballroom of the historic Hotel Oakland senior housing facility. Our sister site, Hong Fook ADHC Center - Harrison Street, is right around the corner at 1388 Harrison Street. If you are interested in a site visit, please feel free to contact us at (510) 839-9673.
